Welcome to the next edition of Soundbites ; the newsletter for Sound Integrative Health and Dr. Adam Rinde. For new subscribers; welcome! This newsletter is intended to serve as a vehicle of sharing and fostering our relationship whether you see me frequently as a patient or have yet to have a face-to-face visit. The newsletter gives me the opportunity to share what I have learned and what I am learning to a group of people that has expressed interest . What is shared here is more of an insider approach rather then what might appear in a blog or on wide social media forums. So without further ado let's jump in.....
Announcements
The emphasis on education, information, and inspiration continues . The past few months have not disappointed.
I had a couple of really interesting podcasts; including one with Jim LaValle on Neuroinflammation , I loved hearing Jim explain how brain inflammation is involved with so many concerns related to focus, memory, moods, and behavior. (More about this later) Also, how the brain and gut interface. He talked a lot about Synapsin ; a B12, Nicotinamide Ribose, and Ginseng nasal spray that I prescribe . It was quite helpful to hear about Synapsin from him since he is the inventor and formulator.
Also, had an interesting and empowering chat on Obesity Medicine with Karli Burridge , Karli shared so many pearls about the actual physiological imbalances in Obesity . She looks at obesity as an energy balance disorder but not necessarily because people are eating too much or not exercising enough. We spoke about Leptin resistance, insulin resistance, orexin, and how the hypothalamus/brain are highly involved with energy and metabolism. She shared some new treatments that are available now for obesity.
We will be having a program designed to gear your immune system and health up for the upcoming Fall and Winter. It's called the 10-day Metabolic clear change program . It is planned to run October 10-20, 2021 with an introductory meeting on October 7, 2021. This is a medically guided nutritional detoxification program designed to reduce inflammation and reverse habits that have accumulated over the summer. Lab to Life
Accumulating information over the course of the month is daunting. The filter I try and run everything through is based on the premises; can I apply this information today for myself, a loved one, friend, a patient, or acquaintance. If the answer is no, not yet, or I am not sure; then the information is filed in the "someday maybe" section or discarded all together. What is deemed useful must first pass that test. So "Lab to life" is just this; information that has come from the lab bench and can be placed in our for benefit today
Adiponectin; A smashing debut
Obesity, fat gain, and weight gain has entered a new era of research. Gone are the days when we are just hearing the old mantra " calories in, calories out" . Researchers are now focusing on the metabolic effects of fat tissue and how inflammatory signaling either helps or deranges our metabolism. One of the molecules that is under study is Adiponectin.
I first wrote about adiponectin in a 2011 newsletter. Adiponectin is a protein hormone (classified as a cytokine) that modulates a number of metabolic processes, including glucose regulation and fatty acid oxidation. Adiponectin is secreted from fat tissue (and also from the placenta in pregnancy) into the bloodstream and is very abundant in plasma relative to many hormones. It is involved with beta oxidation (a fat burning process) and appears to associated with lowering free fatty acids from the bloodstream (lowering cholesterol). It is also involved with insulin sensitization meaning glucose control when explored in women with Type 2 diabetes. I have also learned recently trough IntellxxDNA that its involved with brain neural connectivity .
As we gain weight; the size of of our fat cells (adipocytes) increase in the body. As this occurs, Adipocytes secrete greater amount of inflammatory markers (leptin, tumor necrosis factor, resistin, free fatty acids) which end up increasing insulin resistance and also increase the production of glucose by the liver. A the same time these Adipocytes will decrease the secretion of adiponectin.
So the interest in how to keep adiponectin levels elevated is certainly a topic of interest in weight management.
I recently learned about the SMASH acronym for raising adiponectin ; meaning including consumption of omega-3 rich foods in the diet such as salmon, mackerel, anchovies, sardines, and herring (SMASH). This meta analysis showed that fish oil modestly increased adiponectin levels. Now , this is most appropriate for someone on the brink of type II diabetes or fully into diabetes but really any one who is struggling with obesity should think of this. Of course, there are concerns about how often we should eat fish as some are a source of mercury. Its best to refer to to the Monterey Bay Aquarium to understand the importance of purchasing and consuming fish sustainably harvested. Also this guide to see if which fish to avoid due to mercury content. For my sardine and mackerel consumption I purchase Wild Planet products. Why? For example King Mackerel is on the avoid list; while planet uses Atlantic Mackerel in their product. This is a more sustainable type. I have not really ever been so into anchovies even though my New York City-bred Father, swears that they are amazing as a pizza topping.
I generally recommend 2-3 servings of fish per week from the SMASH list and a daily mercury free fish oil like O.N.E. Omega by Pure Encapsulations. Also worth noting that High Intensity Interval Training (HIIT) also increases adiponectin while lowering inflammatory markers (Il-6, TNF-alpha).
You will hear me lament a little bit about how High Intensity Interval Training should be done in appropriate doses. If done too often in the wrong person I see other problems arise like cortisol imbalances. I do think that a baseline of low intensity aerobic exercise most days of the week and 1-2x's per week of HIIT with adequate recovery might be the most reasonable way to approach things.
Brain Fog
Brain Fog is a common health complaint that spans across many health disorders . This includes autism spectrum, ADHD disorders ,celiac disease, chronic fatigue syndrome, fibromyalgia, M.S., mastocytosis, postural tachycardia syndrome, and early/minimal cognitive impairment. I also see it in SIBO and other dysbiotic scenarios like IBD and IBS,
The common thread is neuroinflammation that is related to heightened microglial activation secondary to excess histamine release . Histamine can be excessive in the body due to genetic predispositions, excess histamine in foods (especially left overs), and excess histamine being produced by dysbiotic gut bacteria.
If unchecked the histamine can lead to increased brain fog and decreased learning and cognition.
The brain is the CEO of the body . If it is underfunctioning then all functions and organs that serve it seem to suffer. Just like any organization. Taking care of the brain is as important as taking care of the heart.
This article looks at the benefit of luteolin in these scenarios. Luteolin is a flavonoid found in many plants that is determined to be inhibit mast cell degranulation . As part of our immune system, mast cells reside throughout our body including our brain, lungs, and gut. When mast cells degranulate they release histamine. Luteolin can slow this degranulation. It also appears to reduce microglial activation and reduce the inflammatory molecule IL-6. As we are getting more sophisticated it appears liposomal forms of this vitamin might work best. I have seen one brand that appears to have liposomal luteolin and its called neuroprotek , The author of this article is Dr. Theoharides who also formulates Neuroprotek. I have no problem endorsing this though as he donates the proceeds of the supplements to research and he has devoted his life to helping patients with adhd, autism, and mast cell disorders.
There are many ways to approach brain fog but my general approach is to look at what is creating the fire (inflammation) and try to extinguish it. But while searching for the fire, we must control the smoke (the histamine).
In my Kitchen
Coffee alternative ; MUD Water is my latest experiment . Not to quit the enjoyable once a day coffee but as a replacement for a need for a second cup. This drink is a mixture of Chai, Lion's Mane, Reishi, Chaga, Cinnamon, Turmeric, and Salt. It is no secret that I enjoy my morning coffee, but more then one cup a day has never felt good to me. Looking forward to the mind supportive aspects of Mud Water. Let's see if I can "acquire" a taste for it.
Also getting ready for currant season.
Last year my son and I enjoyed picking these up at PCC. You can see he "touched it" so we had to buy it. Oh well! The microbiome supportive polyphenols from currants are so supportive for the diversity of our microbiome. We all should partake in these in late July and Early August. Also , freezing for the winter would be a good idea!
Books
Misinformation and Disinformation leading us down a rabbit hole? Seem familiar? The last 19 months have exposed how hard it is to get good information. I picked up a book and am reading a book called Smart Health Choices: making sense of health advice by Professor Les Irwig, Judy Irwig, Dr. Lyndal Trevena, and Melissa Sweet.
While these folks take a swing at my profession quite a bit , I still appreciate how they guide the reader through the ability to understand the basic terminology of research and stats and how to avoid become deceived.
Its empowering actually understand the research enough to make better decisions. Relying on "headlines" is not a great way to make decisions.
Of course, going to medical school we took courses in statistics and research analysis but to truly become an expert in this craft you have to read and analyze a lot of papers. This book helps the layperson and even the busy clinician feel more empowered .

All Heart
Relationships cannot be sustained without presence.
And the presence doesn't have have to be full of roses and shiny things.
This was shared recently and it has made a significant impact on me.
When we spend time (quality or low quality) with another person, thing, or being ; we have a chance at relationship. Without that space and time shared together the relationship suffers or fails to emerge.
There is so much pressure and emphasis that when we spend time with another ; that we have to be "perfect".
Not so.
The act of spending time together gives the relationship a bond.
But without shared time and space together; not much can be done to create that bond.
For example, the parent that consistently picks up their child from school and takes time to be with the child before scurrying off to something else is creating a bond. It doesn't mean the parent has to be happy, cheery, and super fun all the time. Just showing up and being consistent is creating a relationship. Might seem basic but to me its profound.
The presence over time creates closeness and trust.
As they say "showing-up" is half the battle and being "real" is ok.
