When Gut Freedom Has Left The Building
the 5-food prison.
Today am sharing today a common scenario that over the last 20 years I have seen in my practice. And it is one of the inspirations for creating The Gut Freedom Intensive Course and a framework for anyone who is struggling with chronic GI distress.

“Doc, I am down to 5 foods… carrots, chicken breast, seeded cucumber, white rice, and sweet potato…Anything else and I react”
I lean in, and ask
‘What do you mean?’
“If I eat anything else, I get bloated, stomach pain, diarrhea, , brain fog, fatigued, congested, headachy, swollen, and sometimes itchy”
My clinical brain starts to determine if this is an acute or chronic problem, and I ask
‘How long have you been dealing with this?’
“ This has happened before, many times actually. This last time it was after I had a big deadline at work and i wasn’t getting much sleep. At that time I wen to the ER and had was put through every test imaginable including an endoscopy, colonoscopy, ultrasound, blood tests, and stool tests. Now this happens at many times a year. Yet, my gut never feels ok”.
I explain to the patient.
‘I am sorry you are going through this and I describe this as the 5-food prison...
It is a perfect example of when gut-freedom- has left the building.
And, I have seen this happen too many times to count in my career.
Let me tell you the story of what happens sometimes and how we can break free. While there is exam and testing still needed lets just discuss a possible framework…’
I pull up the diagram and explain
‘Imagine your immune system is like a security guard at a building. Normally, it knows the difference between friendly visitors (like food) and dangerous intruders (like viruses). But sometimes, things happen that confuse the security guard, and it starts treating everything even safe foods—like a threat.
This diagram shows how someone ends up in what doctors call the “5-food prison”—where they can only eat a handful of foods without feeling sick.
The Beginning: Three Troublemakers (Left Side)
Everything starts with three problems that damage your body’s systems:
1. Chronic Stress
- Working too much, not sleeping enough, constant worry , (also chronic trauma patterns)
- Your body stays in “emergency mode” all the time
2. Circadian Rhythm Disruption
- Your body has an internal clock that controls sleep, digestion, and immune function
- When you work night shifts, look at screens late at night, or don’t have a regular schedule, this clock gets confused
3. Environmental Toxicity
- Pollution, chemicals in food or products, mold exposure
- These toxins damage your cells and immune system
Notice the arrows between these three? They make each other worse. Stress disrupts your sleep schedule, poor sleep makes you more sensitive to toxins, and toxins create more stress on your body.
The First Breakdown: Immune Dysfunction (Center-Left)
When those three problems pile up, your immune system stops working correctly. This is called **immune dysfunction**.
Think of it like this: the security guard (your immune system) has been working triple shifts with no breaks, dealing with constant alarms. Eventually, they start making mistakes—letting in real threats or blocking safe visitors.
**MCAS Note**: In some people, this gets really severe and turns into something called MCAS (Mast Cell Activation Syndrome). This is like the security guard completely panicking and hitting the alarm button for everything. It makes all the next steps happen much faster and more severely. (see MCAS course)
The Gut Breaks Down: Three Problems (Center)
Once your immune system is confused, three things happen in your digestive system:
1. Intestinal Permeability (Leaky Gut)
- Your intestinal lining is supposed to be like a screen door—letting nutrients through but keeping out bigger particles
- When it gets damaged, the “screen” gets holes in it
- Food particles that should stay inside your gut leak into your bloodstream
2. Dysbiosis(Bad Gut Bacteria)
- Your gut contains trillions of bacteria—and when balanced they are quite helpful
- When the balance gets messed up, there can be too much gas, bowel changes, and inflammation.
3. Food Intolerance, Sensitivity, and Allergy
- Your immune system starts reacting to foods it used to ignore
- This creates inflammation every time you eat and difficulty finding safe foods.
These three problems feed each other (see the arrows?). Leaky gut creates an environment where bugs can grow even more and more imbalances persist. Perpetuating the whole cycle.
The Body Stops Working Right: Five More Problems (Center-Right)
As the gut problems continue, your digestive system breaks down in multiple ways:
1. Malabsorption
- Your body can’t absorb nutrients from food properly
- You might eat plenty but still be malnourished
2. Visceral Hypersensitivity
- Your gut nerves become overly sensitive
- Normal digestion starts to feel painful
3. Immune Intolerance(Loss of Tolerance)
- Remember that confused security guard? Now it’s completely lost its training
- It treats almost every food like an enemy
- This is the key step to the “5-food prison”
4. Dysmotility
- Your gut isn’t moving food through at the right speed
- Food might move too fast (diarrhea) or too slow (constipation)
5. Maldigestion
- Your body isn’t breaking down food properly
- Even simple foods become hard to process
All these problems are connected (see all those arrows?). They create vicious cycles where each problem makes the others worse.
The End Result: IBS and the 5-Food Prison (Right Side)
Finally, you end up with **IBS** (Irritable Bowel Syndrome)—but not just a “waste-bucket” term. This is severe IBS where you can only tolerate a few foods without getting:
- Bloated
- Fatigued
- Brain Fog
- Congested
- Headaches
- Swelling
- Itching
- Abdominal Pain
- Bowel Changes
You might end up eating only carrots, chicken, cucumber, rice, and sweet potato—hence the “5-food prison.”
Why This Diagram Matters
This shows why simply avoiding foods doesn’t fix the problem. You have to work backward:
1. Fix the stress, sleep, and environmental issues
2. Calm down the immune system
3. Rebalance the microbiome
4. Heal the gut
5. Restore tolerance to foods
It’s not quick or easy, but understanding the whole chain helps explain why treatment takes time and why you need to address multiple things at once.
The good news? This isn’t permanent. By working backward through these steps, many people can escape the 5-food prison and enjoy a wide variety of foods again.
